ONWAA is sharing this Ontario Electricity Support Program First Nation Referral Form with Income Assistance Administrators to help support your clients in receiving the benefits of this program.

We are sharing this updated form for those with clients on social assistance. With their permission, you can complete this form and send it to us to process.  Please do not use the old forms and use this SA OESP referral form moving forward when completing the referral for your clients. For those individuals who are not on Ontario Works, a regular OESP form and consent will be required.  For individuals not on Ontario Works, a consent form will need to accompany the regular OESP form.
