Event Details

Case management is the practice of working with individuals to develop a plan that coordinates and integrates support services to help meet their unique needs in effort to achieve Mino Bimaadiziwin – the good life. Case managers service individuals with various needs which requires dedicated skills, knowledge and specialized techniques to ensure supports are reflective of each individuals needs, and that resources are optimized and effective to achieve desired outcomes.

In this session, participants can expect to have conversation and provide input on: defining life stabilization and appropriate supports, designing assessment tools, establishing support networks, and identifying resources needed to stabilize, support, and motivate an individual’s journey from where they are at. 

This 1.5 day session will be held at the Chelsea Hotel in Toronto on December 4 & 5, 2024. A room block is held until November 4, 2024 and is first come first serve. Reservations can be made by calling the hotel at 416-595-1975 and quoting "ONWAA - Life Stabilization and Case Management". 

Lunch is not provided, attendees are responsible for their own lunches.
